Long-term clinical Outcome of Revision Hip Surgery

Our long-term goal is to improve clinical outcomes of revision joint replacement implants by improving their fixation.

We have shown that interventions yielding superior fixation of primary implants do not uniformly improve fixation of revision implants, due in large part to devitalised sclerotic bone rim typically forming during aseptic loosening. The smooth and relatively impermeable surface of the sclerotic bone rim reduces shear attachment strength and act as a barrier to the transport of cells and growth factors. Clearly, revision implant fixation is also complicated by bone defects and loss of structural support.

Here we focus on improving revision implant fixation in the devitalised sclerotic shell forming in the proximal cancellous region. Secure fixation at this site is necessary to obtain long-term implant fixation. Our approach to improve revision revision implant fixation is within the framework of three focus areas: (1) increase access of native growth factors to the interface by disrupting the revision setting’s sclerotic bone (SB) rim, (2) stimulate bone growth directly at the implant surface by implant coating, and (3) augment local concentration of growth factors autologously and pharmacologically.
